The file needs to be called GPUs.txt, but you don't appear to need it. All three of your GPUs are recognized properly by the client so I don't think it's going to change anything.
According to the log that you posted, your FAHClient recognizes 3 GPUs and assigns them to slots 00, 01, and 02.
Enabled folding slot 00: READY gpu:0:"G92 [GeForce 8800 GT]"
Enabled folding slot 01: READY gpu:1:"GF110 [Geforce GTX 590]"
Enabled folding slot 02: READY gpu:2:"GF110 [Geforce GTX 590]"
It then starts three FAHCores on WUs that have already been downloaded
WU01:FS01:Running FahCore: FahCore_15.exe -gpu 1
WU02:FS02:Running FahCore: FahCore_15.exe -gpu 2
WU00:FS00:Running FahCore: FahCore_11.exe -gpu 0
(I've editied out a lot of text that's not part of this issue.)

WU01:FS01:0x15:Project: 8005 (Run 8, Clone 113, Gen 5)

At that point, it's not clear which WUs are actually running and if others are not. You paused and restarted before I got enough information. Nevertheless, all four slots (including SMP) started up normally and some resumed from a checkpoint.
Clearly one GPU is working which is what you reported:

When there is more than one type of GPU in a system, (you have both FERMI and the older G80/G92) it can misidentify the GPUs, giving the wrong type of work to the wrong GPU. I need additional information to know if that's what is going on here.
Please download GPU-Z and run it. It will list each of your GPUs along with a lot of data that may or may not interest you. What I need to know is the order in which it lists them.
